Purpose This dissertation aims to assess the prevailing situation of Healthcare delivery services in rural population and to evaluate further the innovations required to integrate it end to end so that the objectives of equity, affordability, efficiency and quality healthcare are met. For improved patient outcomes and performance of service delivery, innovation around e-health may not be sufficient enough alone until and unless there are complimented by parallel institutional reforms. Hence this dissertation would also provide the foundation data, prevailing landscapes of e-health and highlight the ongoing initiatives and relevant research to be conducted for the implementation of PPP1 models for measuring patient outcomes and PHC performances. In this dissertation we are importantly examining critical success factors for the widespread of e-health and further trying to also understand challenges as well as opportunities regarding the integration of rural health service delivery. I would be eventually proposing a innovative collaborative model for integrating healthcare delivery services after evaluating it against various theories and analytical models. Design/approach Most of the primary data is being collected from the GP or sub-Taluk level. The study is based on literature review and survey s and interviews of persons involved in activities of providing information to the rural community as well as residing in rural areas. A comprehensive research endeavour was started to collect Primary data in 4, GP of Gubbi Taluk,Tumkur district, Karnataka covering various stakeholders and actors such as Physicians in PHC3and PHU4, Physicians extenders(assistants ,nurses),GP secretary, Patients(focus more on child and maternal) to understand the response and perceptions on the need of telecenters (CSC) in their regions and its eventual viability. Also the prevailing health conditions and PHC performances were measured. Primary data was collected around Patient health status, patient demography, patient income group, catchment, socio-economic factors, patient travel time and access to nearest PHC and prevailing PHC/PHU performances using questionnaire as elaborated in [Annexure I].Secondary data was used from Database of NFHS survey III by Government of India. Originality/value The dissertation presents a innovative collaborative model between e-health and PPP at various tiers of the rural divide. This dissertation will bring out the appropriate performance standards to be used in the contracts while framing PPP contracts.. It would eventually help the readers to understand the required scope and implementation of innovations in e-health and PPP model across the rural and urban segments, for religious and rigorous integration of healthcare service delivery for eventual improved patient outcomes.-
